Fee Schedule

• Reports (Incident Reports, Supplemental Reports, Case-Supplemental Documents): $0.25 per page.
• Wisconsin Motor Vehicle Crash Reports (DT4000 Form): $1.00 (flat rate).
• Photographs and videos are sent electronically via email. DVD or CD fees available upon request.
• Bodycam Requests have an estimated completion schedule of 6 to 8 months from date of received request.
• Prepayment may be required for larger requests.
• Our agency does not use "Dash Cam" or "In-Car Cam."
• For 911 Dispatch Tapes, Radio Communication, or Dispatch Call Log Requests, please make your request directly to the Communications Center at this link.

Information About Your Request

• You will be contacted by email once your request is completed.
• By submitting this form, you acknowledge this is an open records request subject to timelines, review, release restrictions, fees, and lawful redactions per Wisconsin and Federal statutes.
• Our Records Bureau will complete your request as soon as practicable and without delay.
• Completion may take longer than expected due to the volume of requests being received and the amount of material being requested per requester.
• An open records request is not a substitute for a Discovery request in criminal proceedings.
